Major Insurance Plans We Accept
Sync Chiropractic & Rehabilitation currently accepts major plans including Blue Cross Blue Shield, TriWest, and UnitedHealthcare. Participation and benefits can differ by plan, employer group, service, and location. If your insurer is not shown, contact us—additional plans may be accepted.
Insurance acceptance does not mean every service is covered under every plan. We recommend benefit verification before your appointment.
How Insurance Works at Sync
A simple three-step process helps reduce surprises and lets you focus on your care.
1. Share Your Insurance Information
Send or bring your photo ID and the front and back of your insurance card. We may also need the policyholder’s name, date of birth, and your reason for visiting.
2. We Verify Your Benefits
Our staff contacts or checks your insurer to review network status, chiropractic or rehabilitation benefits, visit limits, referral rules, and prior-authorization requirements.
3. Review Costs Before Care
We explain the benefit information available to us, including any reported copay, deductible, or coinsurance. Your insurer makes the final coverage and payment decision after a claim is submitted.

Does insurance cover chiropractic or rehabilitation care?
Many plans include chiropractic or rehabilitation benefits, but coverage varies. Your plan may limit the number of visits, require medical necessity, or exclude certain services.
What might I pay?
Your responsibility may include a copay, deductible, coinsurance, or charges for non-covered services. Meeting a deductible can change what your plan pays during the year.
Do I need a referral or prior authorization?
Some plans require one or both. We will review the available benefit information, but patients should also confirm plan requirements directly with their insurer.
What if my plan is out of network or I do not have insurance?
Contact the office. We can discuss available self-pay options and help you determine the most practical next step.
What to send us
- Photo ID
- Front and back of your insurance card
- Patient and policyholder names and dates of birth
- Member ID and group number
- A brief description of the condition or reason for your visit
Please bring your insurance card and photo ID to your appointment even if you submitted them in advance.
